Information Security Officer Contact Information
Introduction
As a part of the Annual Privacy Compliance Review, each LEA must provide USBE with the contact information for the LEA's designated Information Security Officer.
2024-25 Privacy Compliance Review
Will our LEA need to submit our information security officer(s)'s contact information in the 2024-2025 Privacy Compliance Review?
Yes. Even if we have currently have your information security officer's information on file, we need to confirm that the information is correct and that the person we have on record is still acting in that role.
How and when will the information security officer's contact information be submitted?
In September 2024 an email with a link to the Information Security Officer Contact Info Survey* will be sent to all the information security officers we have on file.
What will be asked in the survey?
Below is a list of the information we'll be asking for in the Information Security Officer Contact Info Survey.
- Confirm that you are still the information security officer for your LEA, or provide the name of the new information security officer.
- Confirm your information:
- Name
- Email address
- Phone number
- Job title (aside from information security officer)
- Name, title, email, and phone for the information security officer's supervisor
- Why do we want information about your supervisor? We'll reach out to the supervisor if the information security officer is not responding--usually because they no longer work at the LEA.
- Answer questions about your role (optional)
