New Sponsor Application Training (Please watch the following short video before starting your application process)

Steps Application Process
Thank you for your interest in the Child and Adult Care Food Program. Below are the steps that need to be completed before you can come onto the program:

Step 1—Take & Complete Basic Training

You may choose any one of the two options below to complete the training.

In-person training
Online Training
  • Access the online CACFP Basic Training by going to: https://usbe.instructure.com/courses/427. Follow the instructions on the main page and complete all the necessary training for your program type.
  • The entire training is broken out into 17 different modules each accompanying training videos, quizzes, reflection questions, and activities. You may complete this course at your own pace but please be aware that it will take roughly 5-8 hours to complete all the required training modules.
  • You will receive a training certificate for each completed module. Please print/save a copy of these certificates as your proof of completion.

Step 2.1—Complete Application

To complete your application, you must submit each of the forms below. You may do this by completing them online or by downloading them and submitting them to communityprograms@schools.utah.gov.

Step 2.2—Submit Additional Documentation (if applicable)

The follow documentation will need to be submitted to communityprograms@schools.utah.gov:

  • Food Service Management Company/Meal Vendor Bid and Contract (if applicable) - Contact our office immediately if using, or prior to setting up a contract, with a vendor
  • If Non-Profit: Documentation of 501(c)(3) status, Articles of Incorporation and By-laws, Board Meeting Minutes approving CACFP participation

Step 2.3—Submit Documentation

The following form will need to be submitted only after you have sent everything else and have been in contact with a Food Program Specialist

Step 3—Schedule Pre-Approval Visit

A CACFP staff member will schedule a pre-approval review to verify systems are in place. You will need to have sufficient paperwork and processes  in place learned during training  before approval

Step 4—Approval & Submit Claim

State office will send approval letter and signed permanent agreement to sponsor. Sponsor will be able to start claiming meals

(**IMPORTANT. Our office does not process food program applications during August and September. Preapproval visits must be completed by August 15th in order to continue with the application process. We will follow up in October with all inquiries and applications received during that time. ** )
